Your First Visit
Your first visit
to our office is very important. This is the time for you to
express your concerns and discuss your needs with us. If the first
visit is an emergency, we will of course address that immediately.
This initial
visit normally involves a comprehensive exam and diagnosis and sometimes
treatment can be done the same day as the consultation. However, a
complex medical history or treatment plan will require an evaluation and
a second appointment to provide treatment on another day. During the
first appointment time a complete health history and dental exam along
with necessary x-rays will be taken and reviewed. This visit takes
approximately one hour.
If your treatment plan is
uncomplicated, your options may be discussed at this visit. If your
condition requires more complex planning, a separate consultation may be
needed. There is no additional charge for that visit.
A cleaning
may also be done at your first visit. Depending on how recently you've
had a cleaning and the existing condition of your teeth and gums, we may
recommend further preventive treatment.
Please assist us by providing the following information at the time of
your first visit:
- X-rays,if applicable, taken within the past
year.
- A list of medications you are presently
taking.
- If you have dental insurance, bring the
necessary information. This will save time and allow us to help
process any claims.
- Complete health history and information forms
below
Please alert the office if you have a medical
condition that may be of concern prior to surgery (i.e. diabetes, bone
density medication, high blood pressure, artificial heart valves and
joints, or if you are on any medication (i.e. heart medications,
aspirin, anticoagulant therapy, etc.) or require taking medication prior
to dental cleanings (i.e antibiotic pre-medication)
X-Rays
If your previous dentist has taken recent x-rays you may request that
they forward them to our office. If there is not enough time, please
pick them up and bring them to our office. If additional images are
necessary, they can be taken at our office.
IMPORTANT: A
parent or guardian must accompany all patients under 18 at the first
visit.
